Here you will find available applications and forms:
- Activation of the USiCard for library use
- Form for students of the University of Siegen
(For visiting students and students who do not wish to have their USiCard activated for library use) - Library card for Non-University library users
- Declaration of consent (Adolescents under 18 years of age from their parents or legal guardians)
- Library card for University staff
- Application for an extension of the validity of the library card (University staff)
- Proxy for the library account (Only for lecturers of the University of Siegen)
Issuance or renewal of a proxy for the library account
